As Required or Permitted by Law

We may use or disclose health information when permitted or required by law, including for:

You may revoke an authorization in writing at any time, except to the extent that we have already acted in reliance on it.

Your Health-Information Rights

Obtain a Copy of Your Records

You may request to inspect or obtain an electronic or paper copy of health information maintained in your designated record set. We may charge a reasonable, cost-based fee as permitted by law.

Request a Correction

You may ask us to correct information you believe is inaccurate or incomplete. We may deny the request in certain circumstances, but we will explain the reason in writing.

You may ask us not to use or share certain information for treatment, payment, or healthcare operations. We are not required to agree to every request.

If you pay in full out of pocket for a service and ask us not to disclose information about that service to your health plan for payment or healthcare operations, we will honor the request unless disclosure is required by law.

Receive an Accounting of Disclosures

You may request a list of certain disclosures of your health information made during the period permitted by law. The list will not include every type of disclosure, such as many disclosures made for treatment, payment, or healthcare operations.

If you have authorized another person to act for you, or if someone is legally authorized to represent you, that person may exercise your rights and make choices concerning your health information as permitted by law.

Your Choices

When legally permitted, you may tell us your preferences regarding sharing information with family members, friends, or others involved in your care or payment for your care.

If you are unable to communicate your preference—for example, during an emergency—we may share information when we believe it is in your best interest and permitted by law.
